KEY COMMUNITY-BASED SERVICES

Care Coordination

Each participant receives a needs assessment — clinical and social — and is matched with a dedicated care coordinator who develops a personalized care plan.

Clinical & Non-Clinical Care

Pregnancy and postnatal care, doula services, mental health and substance use screening, intimate partner violence screening, and immunizations.

Health Education

Evidence-informed classes on parenting, fatherhood, infant nutrition, and newborn care — designed to build lasting family health literacy.

Social Support

Transportation, housing assistance, and active linkage to community services that address the social conditions shaping long-term health outcomes.

Family Involvement

Fathers and partners are actively engaged throughout the program to strengthen the family unit and improve maternal and child health outcomes.

INTEGRATION WITH FAMILY PLANNING

InKnowGrowth's community-based model complements the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) program. While distinct in structure, both address maternal and child health through coordinated, family-centered approaches.

Reproductive health services are brought directly into underserved communities, reducing barriers to timely, appropriate care.

Addressing Social Determinants
Family planning is linked with housing, transportation, and economic support to drive meaningful, long-term health improvement.

Resource bags are curated bundles of essential items distributed to mothers and infants during prenatal visits, home visits, and community events. They serve as engagement tools, educational touchpoints, and tangible expressions of support.

MATERNAL HEALTH OUTCOMES
Clinical Monitoring at HomeBags include thermometers, blood pressure monitors, breastfeeding supplies, and postpartum depression screening guides to support health monitoring between visits.

SOCIAL DETERMINANTS
Addressing Non-Clinical BarriersTransportation vouchers and tangible support items help remove access barriers while building family trust and resilience in the care relationship.

HEALTH LITERACY
Culturally Appropriate EducationLinguistically appropriate materials on nutrition, safe sleep, breastfeeding, and family planning — plus QR codes linking to video tutorials and community resources.
