Every new participant begins with an in-depth interview covering both health and social needs. From there, we match you with a care coordinator who builds your personalized plan and walks alongside you every step of the way.
A thorough one-on-one conversation to understand health history, living situation, family dynamics, and personal goals.
We evaluate both clinical and social determinants — chronic conditions, mental health, housing stability, nutrition, and community connections.
